On Your Initial Visit:

On your initial visit, please be prepared to share your medical history with our physicians and bring the following items:

• Your insurance card
• Your referral, if required by your insurance company
• List of current medications (prescription or over-the-counter) including:
  • Name
  • Dosage
  • Frequency
• List of allergies you have to medications or foods

Please download and complete the patient forms that are appropriate for you and bring them with you.

Insurance and Payment Information

We accept most major insurances and Medicare. We encourage all patients to confirm their insurance coverage, deductibles and co-pays prior to making an appointment with our practice.

Please bring your insurance card, identification, and referral (if necessary) to your appointment. We also request that you arrive early to your first appointment to complete a medical history; please bring a list of all current medications you are taking to this appointment.
